does it work with win8.1?

Yes, and no. It can be messed with via DL'ing certain things that cause the game to run- but it has to have the latest updates for the pirates and nobles to work. That being said, EA's latest update has a game breaking bug that will cause Sims Medieval to crash and mess up. That was several years ago, and they are not going to fix it because the game itself is no longer 'popular'/ making much money. You might be able to find some downloads that could help, but I could not tell you which ones are safe to run. I tried for 3 days before and lost a good 50 hours of progress in the game before I came to terms that EA stole from me. After I got the base game done, figured out a work around the updates thing and started to play- it would literally delete my characters as I played. Several times it deleted my king or queen as played other characters and I could literally not continue the game at all. My advice would to be to skip the entire game completely, there will be heart ache either way. If you are really determined, you could go for it and maybe you will yield better results than me- Just be prepared! Hope I helped!

this is a disc but can I download it?

I am sure the medieval company has a download for sale on their website, for most pc games company do. But weather or not it will sync with your systems I advise looking really closely to what your system uses and what the download will work on. Best of luck!

It says I need the sims medieval installed. I thought this was the sims medieval! Also, it's in some other language?

This is an expansion pack called Pirates and Nobles for the base game The Sims Medieval. Contact the seller about it being in a different language, especially if you think it won't work. Hopefully they can exchange it for you or at least give you a refund.
